New service - Acute Illness Team
Primary Care Practitioners / Nurse Practitioners
Conditions suitable for assessment by a PCP include cough, sore throat, ear pain, rashes, plus all minor injuries and acute illnesses (acute illnesses are those which started no longer than 2 weeks previously).
Our Primary Care Practitioners (PCP), Steven & Rob, along with the Nurse Practitioners (Tracy and Katie) work alongside the duty doctor every day running the 'Acute Illness Clinic'. They are experienced clinicians specifically trained to assess a patient's condition and offer appropriate advice and treatment to any patient aged over 6 months old. They are also able to visit patients at home should their condition prevent them from leaving their home.
Our Acute Illness Team can be seen 'on the day' by ringing the appointments line - the receptionist will check that a patient's condition is suitable to be assessed by a Practitioner and will offer the patient a booked appointment time. The duty doctor will still be available should the Practitioner feel that GP input is necessary.
Dr Elder's retirement
Dr Elder retired from the practice on 31st March 2017.
All patients who were registered with Dr Elder and had her as their 'Named GP' have been transferred over to either Dr Padmore, Dr Bomford, Dr Hackney or Dr Wong.
These patients do not need to do anything and there is no change to the way they access General Medical Services at the Hereward Group Practice.
South Lincolnshire GP Alliance
GP practices in the South of Lincolnshire have formed an alliance which will see us work more closely on the provision of services for our patients.
This new alliance will give those member practices taking part the opportunity to explore new and innovative ways of providing the highest quality safe care closer to our patient’s home.
Patients at our practices will remain registered with us and with their own GP. Patients will see no change to the day to day care that they receive. It is anticipated, however, that one of the benefits to our patients will be the ability to access a wider range of services locally or even in their own home.
Potential initiatives being considered include extending GP opening hours and taking a more collaborative approach to weekend working. This could see our patients being able to access a GP not necessarily at this practice but at one of the other practices who are part of the alliance.

Linking practices together will enable us able to begin the process of developing cutting edge and evidence based care closer to the patients’ home. The size and diversity of this alliance shows our commitment to work with everyone who has a role in the delivery of health services in this community.”
